Thank you for applying, in order to approve your application we will need identify you.

Please provide one of the following forms of identification:

What ID is acceptable
Driver's licence
Ontario health card (version with address on the back)
Ontario Photo ID card (ages 16 and up)
CNIB ID card

If you don't have any of the above, you must present one piece of identification from the Name Identification list and one piece of identification from the Address Identification list. Expired documents or self-written name and address are not acceptable. Bills, leases, and legal documents must have been issued within the last two months of the date of registration or renewal.

Identification images to use library services are retained until the image has been verified by staff. Upon verification of the identity of the individual, images of any ID provided will be deleted forever to ensure privacy and security of the person:
